Duties
The OT Case Manager-Interventionist provides evidence-informed occupational therapy services, including assessment, planning, and direct interventions, to support client independence and health goals within a community-based setting. This role involves collaborating with inter-professional teams and clients/families to manage care for acute, wellness, and rehabilitation clients in Rockyview County.
Hard requirements
- Completion of bachelor's degree in Occupational Therapy from an accredited university program
- Registered with the Alberta College of Occupational Therapists (ACOT)
- Three years of recent home care or acute care experience in the last five years
- Proficient in computer use
- Knowledge, skill, ability and willingness to work to full scope of practice
- Ability to use critical thinking skills
- Experience working with acute clients and/or long-term clients with a focus on chronic disease management
- Experience in providing case management and interventionist services
- AADL Authorizer
