Service Coordinator - Urgent Response

Duties
Service Coordinator, Urgent Response Services provides service navigation, planning and support to families with infants, children and youth with mental health concerns, autism and developmental disabilities from birth to 18 years. The role involves connecting with high-risk clients, developing working plans, and coordinating cross-sectoral teams to enable timely connections to needed services.
Hard requirements
  • Bachelor?s Degree in Social Sciences, Health Sciences, CYW or Social Work.
  • At least 1 year of related experience working with children and youth with developmental disabilities and their families, with a focus on community engagement.
  • Extensive knowledge of mental health disorders diagnoses and developmental disabilities.
  • Extensive knowledge of other community resources that support the social determinants of children, youth and families.
  • Ability, ease and comfort in engaging and coordinating cross sectoral teams when necessary.
  • Demonstrated experience working from Anti-Oppressive and Anti-Racist Framework and Bio-Psycho-Social Model.
  • Cultural sensitivity and an openness to consulting with and bringing in relevant cultural supports as/ when needed.
  • Knowledge of specialized resources i.e., Transitional Age Resources, etc.
  • A Vulnerable Sector police check.
